Did anyone ever get a forward that sends answers back to you. I am searching for that one now.?

I had once got a forward from someone that took me to a webpage where I had to fill in some information. This was sent to the person who forwarded the mail to me.Likewise when I sent this forward to my friends , their answers came back to me.Was searching for this.Has anyone come across this ?
